B中间的which指代的是什么呢？OG解释说是the effects of drug and alcohol abuse.是不是就是effects?如果这样，和大全的that指代最近的那个词是不是不一样呢？

C中with，解释说不能说明是effects产生了cost. 是不是with优先修饰最近的名词啊(drug and alcohol abuse)？可是with有跳跃修饰的功能的，它可以跳跃修饰谁呢？那些情况下会有修饰对象不明确出现呢？
