impose和require两个动作是并列的，spend是对see more patients的补充。理解句意是关键！

Meaning：工會會員比非工會會員更不可能去參加低檔的保險計劃，這些計劃對醫療服務強加了嚴格的限制，以及要求醫生看更多的病人，治療每個病人的時間更少了。

【要點】
1、根據meaning，可以確定計劃強加和要求兩個動作是並列的，所以impose和require的形式應該一樣；另外，spend time應該是次於前二者的，所以spend的形式應該跟他們不同，另外再根據基本語法特點可以排除ABCE。
2、each指的是patients。
3、spending修飾的解釋
Among lower-paid workers, union members are less likely than nonunion members to be enrolled in lower-end insurance plans[that impose stricter limits on medical services and require doctors[to see more patients]], spending...
in this case, the COMMA -ING modifier could grammatically modify either the blue action or the purple action (which is nested within the blue one).
From context, it should be clear that the modifier is meant to modify the purple action.
(This is normally what happens in this type of situation with nested actions（嵌套的動作）: an attached COMMA -ING modifier will normally modify the embedded, smaller action.)
the COMMA -ING modifier modifies the purple ACTION ("see more patients"), and also applies to the subject/agent of that action ("doctors").
Present participle with comma can modify actions; those actions can be represented by infinitives, etc. as well as by whole clauses.

【解釋】
D、spending表示doctor see的結果，修飾動作-see，apply to the agent（施動者）-doctor.
E、計劃require醫生see more doctors同時spend...錯，因為可以確定require對應的agent是doctor，由於require和spend時態相同，所以默認二者的doer都是doctor。但這樣意思錯！
