Question #1:  Do we say "first of all, ... secondly or second..."?


#2: for, first of all, failing to replace outmoded voting machines prone to breakdowns, and second, for failing to...


I feel that the first "for" is not in the right position - should be: first of all, for failing to replace outmoded...


Please confirm.  Thanks
