The argument says that the doctor gives wrong information to one-fourth of the patients because the doctors wants to earn money from these patient even though the surgery would not help them.
E says that the doctor cannot tell whether a patient will benefit from the surgery BEFORE operating on them. Thus they not motivated by money when telling those one-fourth patients that they need a surgery.

the question requires us to find the undermined choice, that means the the doctors were not interested in an opportunity to practice their skills and in their fees than in helping the patient, the doctor indeed wants to help the patient
the choice E says they teat the patient with inditinguished medicine, that means, not interested in the fees , however, choice D is passed out only because the article doesn't mention the risk ? But it does~~the risk is 1/4.......sorry~~~ maybe I am in the corner and can not get out,,,but ~~~I really quite understand this question>>>>>...... Thank you ~~
