后半句讲的没懂：indicate that it is present when it is not.
BY解释为 when it is not 状语从句承接其修饰对象 it is present 省略了重复的形容词present.

他想说啥啊？

我觉得的意思是
有的时候医学实验没有能够检测出it存在的条件，and 在it实际上不存在时显示存在
不知道对不对？

任何的医学检查都不免有时失败的检测不出存在的病症，也会误诊病症的存在

一些医学测试不能够检测出当it存在的条件也不能说明it显示出现但实际it没有出现
