我觉得这个题的结构是这样的:Among lower-paid workers, union members are less likely ...to be enrolled in lower-end insurance plans that impose... and require...
 这道题要根据逻辑含义来判断,题目含义是:在低收入的工人中,工会成员比起非工会成员更不可能涉足lower-end insurance plans ,这些plans对医疗服务有更多限制,而且要求医生给更多的病人诊治,从而缩短了分给每个病人的时间。
结构中,to be enrolled是没有并列词的,that引导plans的定语从句,从句中的谓语是impose和require,这两个词需要时态相同,spending引导结果状语,作为前面require doctors to see more patients的结果。

Grammar-wise, E means:  (Plans) impose stricter limits on medical servise.  (medical services) requiring requiring doctors to see more patients and spending.......  
However we all know services could not be the logical subject of spending.

楼上的那个句子结构如果那么划分的话,很明显spending的逻辑主语变成了medical services,而spend的逻辑主语应该是doctor,所以可以排除这个选项。

关键就在这个“从而”上了

仔细看这道题 - enrolled in xxx plans that impose, require, spend. plan做的动作是impose, require, 而spend在这里解释plan很牵强。但是spend对之前require doctor之后的结果作说明有明显联系。所以这题就是一个提醒,拿到题的时候还是要注重语义,否则光看平行,直接就xx.
