我觉得前面的中心词是Japan，而不是elderly people，所以选择国家的对应。

老人如何对应呢？谁能给个解释？

为何？是因为japan在前面，people在后面，更靠近than吗？

从句意上，我的理解是japan对应于西方国家，所以选了A。
