I made a similar mistake, the ets wrongly sent my toefl score to the phd program of chicago. I found this after the deadline. So I just called and mailed to the Ad Com. They are very nice and everything settled down within several days and I still meet the deadline. So I think you may tell them the situation if it is already several days after the deadline when you find the mistake and try to get their understanding.
