I consider A the key.
B. intent of doing sth is not idiomatic. The right idiom is "intent to do sth". "at least" should modify "one nurse"
C. It is the state not rules that intends to do sth. Patients not the hospital are the object of "put through triage".
D. "intent of doing sth" is not idiomatic. "ensure that subject + should +verb" is wrong. The clause following ensure is usually in the present simple tense. "in a hospital emergency room" should be put behind "triage".
E. "this" refers ambiguously. The loaction of at least.
