完全可以当作是主语的修饰语.且逻辑主语与主语一致.
再者状语,副词,形容词在句中的位置,对于句意的影响实在是太灵活的说

如 He lives near the Institue只能翻译成他住在学院附近而不能说这里的介词near在动词后就是修饰动词的
我想如果从这点看D中as part of their therapy放的离其修饰词太远也是它错的一个因素

as part of their therapy的逻辑主语本来就应该是young polio victims
应该是practice falling as part of their therapy：练习falling 作为他们治疗的一部分
而不是young polio victims 年轻的小儿麻痹患者作为治疗的一部分

我想介词短语放在句首有两种情况,一个是做定语修饰主句主语
一个是做状语修饰全句,说是修饰全句,但最重要的是修饰全句的谓语动作,对于这点E中as介短的位置放在falling后和放在句首是一样的,并不存在紧随着就表示修饰的好，相反很多时候介短做全句状语喜欢放在句首，因为它起到强调作用再一个就是可以解决歧异修饰
而这里as不能看成是做主语定语的原因就诚如ＧＧ所说，因为逻辑上说不通
至于ＧＧ说的关于介短不能在句首做状语的问题，举一个前辈ＮＮ说过的简单易懂的例子

我觉的放在句首和句尾意思还是有差别的。举个例子
as a teacher ,he often plays football.
你改成 he often plays football as a teacher意思就变了嘛。
而且这里的as a teacher明显也是修饰he不是修饰全句呀！

小弟拙见：
E有一个错误就是在主句主语出现前连续给出了两个介词短语使得句子意思不清楚了。（OG上有好几个这样的解释，但是一时想不起来是那几题了。）一般说来看到一个介词短语提前，我们自然会期待后面主句的出现，如果后面还是一个介词短语就不符合习惯了。相反，D选项直接从主语开始句子，通顺流畅，一下子抓住要点。
仔细看五个选项就可以发现其实是换汤不换药，无非是把as提前造成修饰错误和把during提前（也即with压后）造成歧义。至于during放在falling后面我觉得是没有歧义的，一来本身就是在50s采用的这种方法；二来如果说during要修饰前面的动词，那就应该把during提前放在那个动词后呀，比如说live during或者live with... during；三者放在主句谓语后面的时间状语也很容易理解成修饰主句谓语。
同时as除了修饰victims的歧义外还有一个问题，their在victims之前出现也造成理解上的不便（虽然不是理解障碍）。
