I only heard about the request form for transcripts, which is not important. so you can just ignore it.
but the request form of reference, never heard about that. if you mean the evaluation form, you can ask your recommender to fill it and mail back along with your recommendation letter.(remember treat it like the recommendation letter)or you can just ask your recommender to write a general letter for you and ignore the request form.作者: diudiu 时间: 2003-11-5 09:53
