In Kantovia, physicians’ income comes from insurance companies, which require

since there are a lot of methods availabled , surgens have to think about which one is the best and minimize deviate from the benchmark established by insurance companies.So surgen have to take more time to choose in terms of the principle by insurance company and then the periods for the patient treatment decreases ,and then the descrpeny occours above.

规定会减少时间的,但实际上没有减少。

WHY?有他因阿,医生的活更多了,have a wider range of options in diagnosis and treatment

B好像跟前提矛盾 前提說十年前醫生有較多的裁量權
我選D 病人比較信任醫生 因此願意花較多時間在看診上
有人可以告訴我為甚麼B對D錯嗎?

个人看法:
题干的意思:
保险公司要求医生写很详细的文档同时注意不要偏离公司的治疗准则.而十年前不是这样,那时医生有更多的自主权.于是医生抱怨这样一来没有足够的时间去治疗病人.而实际情况是现在医生平均花在每次治疗上的时间在过去十年中是增长的.
其实这种医生个人的感觉和实际情况的差异可以有很多解释,但落脚点应该是"花在每次治疗上的时间是如何增加的.",选项中只有B直接而有明确地回答了这个问题.

因为保险公司要求医生记录治疗方案并解释其选择的方案同原则有何偏差(保险公司给医生发工资,偏差多且理由不充分,就影响收入咯), 那么当医生有很多的检查/治疗方案选择的时候,他就要花多点时间去一项项的选择分析来决定最佳的方案.

可能是我想多了,我觉得要解释的另一个方面是为什么平均在一次office visit上花的时间变多了,(B,可以解释医生感觉看病时间变少了,),医生OPTION多了,跟office visit有关系吗?

B选项意思是:

 现在的医生在开处方之前一般会帮病人从很大范围的诊断和治疗中选择一种,那医生就会觉得他们治疗病人的时间减少了,但实际上他们还是和病人呆一起的(算在office visit统计的时间里)。矛盾就是这样来地

GMAC的出题人的思维也并不是那么严谨,此题只可能B项是答案,但此项仍有疑点:医生和病人一起选择治疗方案,难道就意味医生会觉得时间少么?简单说,一起讨论方案,有with you这个关键点,表明了医生并非是单独在考虑问题,实际上也是一个交流互动的过程,那么既然如此,为何医生会觉得时间少呢?

可是如B所说,为什么 Physicians today typically have a wider range of options in diagnosis and treatment to consider with the patient before prescribing.一定意味着时间的增多?即导致 the average amount of time a patient spends with a physician during an office visit has actually increased somewhat over the last ten years?

觉得这有个gap在里面……

从严格备案、遵守,偏差要报告到自主权,应该来说这样会使得医生和病人每次问诊的时间更短(注意是during an office visit)
那么,更多的选择去choose from就解释了这个paradox
E选项trap,因为pay physicians a set amount for each office visit ,从而to see as many patients as possible,也就是说时间越短看得越多越好,事实上和argument相反。
