我觉得虚拟语气讲的通，因为原句说如果应该避免duplicated facilities ，那么建医院就是浪费。就是假设了“应该避免duplicated facilities”这个说法成立。如果duplicated facilities避免不避免无所谓，那么建医院就不算浪费。

另外补一点，if only有两种意思，表示仅仅因为（就算没别的原因），或者表示强调意愿（这时if 和only可以拆开来）

even if for no other reason than (本题的意思）

e.g. Willy would have to tell
George more, if only to keep him from petering.

Media studies is regarded as a more exciting subject, if only because it's new.

2. expressing a strong wish

e.g. If only he wouldn’t eat so
noisily.

If he would only learn to eat
quietly.

If only I had listened to you.
