IF I were you, I won't spend so much time on pursuing a phd education in the universities of such level, and you did have a MBA degree from a US instituion.
From my point of view, why not try some better universities, like northwest univ, stanford univ, ucla, UNC. I am not sure whether you have much marketing experience, but I guess you should be clear what your research interest is and what your career goal is.
So, I strongly suggest you reevaluate your school lists and do some change on it. I hope you have a better result and your background does deserve better ones.

我的问题是我的数学只有一点微积分基础，到了第一档的学校，我怕太吃力。。。会不会这样呢？？？
比如说我看了univ of Washington 的课程计划，他们要求 the following as prerequisites for the research methods classes. Those calculus classes are not the first level calculus!!!

uw is surely below top 20, it should be the third level

Exactly!Since LZ have good work experience and local MBA degree,why not try best?Northwest is the No.1 in marketing field,and has good reputation both in domestic and in international industry.
Math is never a problem for Chinese, even though you have no idea about the calculus stuff, I believe you can pick them up quicker than Americans.
Don't hesitate to reevaluate yourself again and again, school's choice is more important for phd comparing to mba.
