选E，但我觉得都不对，因为句中是for， first of all， failing to，所以我觉得如果表并列的话应当把E选项的for去掉

另外，second可以替代secondly吗？

首先，first of all 和second都是插入语。

去掉这两个插入语，就能看出句式的平行了：

A recent New York Times editorial criticized the city’s election board for failing to replace outmoded voting machines prone to breakdowns, and for failing to investigate allegations of corruption involving board members.

再回答第二个问题：
如果前面用firstly, 后面就用secondly；如果前面用first (of all)，后面就用second，需前后保持一致。

there are two kinds of failure mentioned in the sentence, so they should be parallel.

for failling to replace..... and for failing to investigate....

ABCD are out.
