A is wrong b/c pilot error is in fact the factor, but the argument says that this factor can be caused by other factors such as design, training, etc.

D is right b/c the argument says that only  the investigators can do something to make sure that the same type of error does not recur and cause another accident.

Although I think D is better--investigators supervise the three factors may contributed to future accidents, I will chose E--comprise one factor triggering future accidents-- if D is not an option.
