I did a little bit more research on this topic, and here are my impressions:
- If you can "manage" your recommenders to closely follow the schools' instructions and answer their questions one by one, it's great -- it demonstrates once again your are managerial material
- If your recommender don't have time etc. to fill in the blank answering all the questions in detail, it should still be acceptable to submit one cover-all letter of recommendation. Schools, hopefully, won't hold this against you too much.
- There are actually some benefits of a complete LoR: it can be written in a more persuasive/powerful way showing holistically the applicant's qualities, comparing with the piece meal answering the questions format.
- Bottom line: don’t force your recommender to do things s/he doesn’t like to do. Don’t argue with them (although you can politely try to persuade them). Make them happy. Otherwise in the end you will become unhappy :-)
